Cook Children’s Health Care System is seeking a Registered Nurse (RN) Case Management for a nursing job in Fort Worth, Texas.

Additional Information:

Performs case management/service coordination activities that enable Cook Childrens Health Plan members with chronic conditions or complex needs to achieve an optimal level of health and independence through advocacy, assessment, planning, communication, education, resource management, care coordination, collaboration, and service facilitation. This is an office-based position with required field work. Driving and in home visits are required up to 75% of the time. Some weekends may be required to meet member needs.

Education and Experience:

  • Requires a Nursing Diploma or Associates Degree in nursing. Bachelor of Science in nursing strongly preferred.

  • Three years of clinical experience.

  • Two (2) years of experience working with vulnerable populations and people with chronic or complex conditions preferred.

  • Strong skills in medical assessment/medical record review and/or behavioral health assessment.

  • Prior MCO experience a plus.

  • Must possess the ability to function in an interdisciplinary, team environment.

  • Self-starter with the ability to be effective in a large, complex and highly matrixed organization is a must.

  • Need to possess the ability to focus on both the details and the big picture simultaneously.

  • Bi-lingual (English/Spanish) Preferred. No testing requirement.

Required skills: relationship-building, effective written and verbal communication skills that allow clear and concise presentation of conclusions and recommendations to a variety of audiences, critical thinking and problem-solving, ability to plan and organize effectively, proficient in typing and in use of computer applications (Excel and Outlook).


Certification/Licensure:

  • Registered Nurse current, unrestricted Texas license.
